Remove/Hide List title from a form

I wish to remove the list title from my form (see link). How do I go about doing this?

Just remove the corresponding control from the form in Forms Designer.

I guess "ssteinig" refers to remove the list name that appears at the top of the page, not the field "title" of the list.
I would like to do so, as well the bottom section with the created/modified timestamps
Is that possible?

You can hide the bottom section by enabling 'Buttons' feature and removing the appropriate control from the form:

As for the list name, could you provide a screenshot?

When you create a form in SharePoint, it will typically have a title that is displayed in the ribbon and in the form title bar. This title is automatically generated and can be changed if you want.

If you want to remove the RemoveHide List title from a form in SharePoint then open the form in question in SharePoint. and click on the RemoveHide List Title button.
