New form malfunction

I have three roles All Users, DataEntry, Owners. I have different New, Display, Edit forms based on the role. The Edit and Display forms work as expected. However the same New form (All Users) appears for the other roles. I have deleted all the forms from SharePoint Designer and republished them, but still only the one form appears for the other roles. The new form for All Users doesn't use the tab control, but the other roles use the tab control. I have checked the default and it is correct. When I open the New form controller aspx there is an error "Error Creating Control Request is not available in this context.  Any help would be appreciated.

    [#12]: I have four groups. All users, Designers, Coordinator and Managers. These are based on SharePoint groups.

    Have you customized the list view and the New item link in particular? Could you provide markup of the link (onclick and href attributes)?

    [#14]: I haven't modified anything. How do I get this information for you. I opened the view in SP Designer and can't find the +new item image or link..

    [#17]: Please, follow the steps:

    1. Remove all form files beginning with 'fd_' via SharePoint Designer.
    2. Re-save the forms in Forms Designer

    If it does not help, pay for the support minutes:

    And provide temporary access to the site with the site collection admin permissions.
